What this case tests

Show the learning points (spoiler: read after you have practised)
An unexplained oral ulcer persisting >3 weeks — non-healing, with indurated (firm/raised) edges and bleeding, on a high-risk site — is suspicious for oral cancer and must not be reassured away as a "denture rub" (a denture ulcer should heal within ~2–3 weeks once the cause is adjusted). The NICE NG12 action is to consider an oral cancer suspected cancer pathway referral for ulceration lasting more than 3 weeks — and with these features, to make it — and a persistent unexplained neck lump is itself an NG12 criterion (oral cancer, and laryngeal cancer at 45 and over) — with the ulcer it raises concern for nodal spread, not "just a gland". Examine and PALPATE in person (induration, neck nodes) rather than relying on a photo, and weight the risk factors — smoking, alcohol (which synergise) and betel/areca-nut/paan (an independent carcinogen). The examinable hidden agenda is fear-driven denial: a brother who had head-and-neck cancer, with dread of treatment/disfigurement, reframed as a benign denture sore, plus possible cultural reluctance around betel. Surface the fear, treat the wife’s prompting as the reliable signal, address risk factors without cultural judgement, balance honesty with reassurance, and complete the suspected cancer pathway referral with documentation and follow-up.
