What this case tests

Show the learning points (spoiler: read after you have practised)
A firm, PAINLESS lump arising from the body of the testis is testicular cancer until proven otherwise — and painlessness is the concerning feature, not a reassuring one. The NICE NG12 action is a suspected cancer pathway referral for a non-painful enlargement or change in shape or texture of the testis, with examination and an urgent scrotal ultrasound — the separate NG12 direct-access ultrasound route is for unexplained or persistent testicular symptoms — never phone reassurance; an undescended-testis/orchidopexy history is a genuine risk factor. The examinable hidden agenda is that the young man’s bravado ("my girlfriend made me", "it’s fine, right?") masks real fear — he has googled it, seen "cancer", and is specifically terrified of losing a testicle, his fertility, and his masculinity. The skill is to refuse the dismissal without crushing him, surface the fear and the girlfriend’s reliable insistence, examine and refer on the urgent pathway, and meet the masculinity/fertility fears with accurate good news (testicular cancer is highly curable; the remaining testis preserves function; sperm storage and prosthesis exist) so he engages rather than delays.
