What this case tests
Show the learning points (spoiler: read after you have practised)
A very high ferritin with high transferrin saturation is iron overload — most likely hereditary haemochromatosis — and the constellation often hides as "ageing": fatigue, MCP arthralgia, bronze skin, reduced libido/ED, dysglycaemia. It is common, treatable with venesection, and good outcomes follow early detection. The examinable core is the hidden agenda: the patient’s distress is not his own health but GUILT about hereditary transmission to his children, shadowed by a father who likely had it undiagnosed. Handle the genetics accurately (autosomal recessive HFE; first-degree relatives offered genetic + iron-study cascade testing) while actively reframing the guilt — he could not have known, and diagnosis means his relatives are caught early and protected. Address alcohol proportionately (it compounds liver injury) without reducing the diagnosis to drinking and repeating the family’s mislabelling, assess organ impact (formal fibrosis assessment, as ferritin is above 1000 with raised ALT — EASL 2022), note that HbA1c 47 is non-diabetic hyperglycaemia rather than diabetes (NICE PH38), and offer 6-monthly HCC ultrasound surveillance if cirrhosis is present (NICE NG50).
