What this case tests

Show the learning points (spoiler: read after you have practised)
A PSA request is an informed-choice consultation, not a reflex blood form or a paternalistic refusal. The core task is correcting the "all-clear" misconception with balance: PSA is non-specific (raised by BPH, infection, ejaculation, exercise), yields false negatives, and over-diagnoses indolent cancers, leading to biopsy harms and over-treatment — so a normal result is not "in the clear". The genuine informed-choice discussion lays out benefits and harms and supports the man’s right to decide; asymptomatic eligible men can request PSA after counselling. Risk factors must be weighted honestly — Black ethnicity (higher, earlier risk) is a legitimate part of the discussion; an uncle is a second-degree relative and adds little, whereas a father or brother, or a BRCA variant, would matter. BRCA2 carriers are high risk and have targeted surveillance through specialist or genetics pathways, not population screening; the Prostate Cancer Risk Management Programme applies from 50. The examinable hidden agenda is anxiety triggered by a colleague’s diagnosis plus an unspoken fear of downstream treatment harms he hopes a quick normal test will let him avoid; surface and address it. Screen briefly for symptoms/red flags that would convert this to a symptomatic NICE NG12 pathway, give pre-test practicalities, and safety-net the result and follow-up.
